*Oversee the transition of patients from hospitals’ specialist clinics to primary and community care.
*Coordination, monitoring and tracking of patients to ensure continuity of care upon discharge, and will collaborate with a multi-disciplinary team of doctors, nurses, medical social workers and para-medicals from the clinics to ensure the smooth transition and right sitting of patients to primary care.
Requirements
• Diploma in Nursing
